AHCCCS & Medicaid Plans
Premium Legacy Healthcare is actively contracted with select AHCCCS plans and continues to expand access through additional Medicaid partnerships. We are currently accepting eligible members through approved AHCCCS networks and are pursuing additional contracts to better serve Arizona residents.

Self-Pay Options
For individuals without insurance coverage or those who prefer not to use their insurance benefits, we offer affordable self-pay options for many of our services. Our team can discuss available rates and payment arrangements to ensure treatment remains accessible.
Verification of Benefits
Insurance coverage does not guarantee payment. Patients are responsible for any applicable copayments, coinsurance, deductibles, non-covered services, or denied claims as determined by their health plan. We encourage all patients to verify benefits prior to receiving services. Our staff is available to assist with benefit verification and authorization requirements whenever possible.
